Set of Four Letters

How many set of 4 letters that can be changed in order ( so something like b-o-o-k and k-o-b-o) are there if they are used to fill in the blanks and made a sentence that make sense?

A _o_a_l_ doctor had _o_a_l_ and therefore was _o_a_l to operate.

If I understand this, the words should fit "?o?a?l?". And the same 4 missing letters are swapped to make the sentance.

'Bent' iss only a partial answer. The question was how many sets of 4 letters can be used. It doesn't even require the sets to be a word.
